Documentation of completing the activity will be kept with each student’s permanent file. If the 0.5 credit has been waived, then it will be recorded on the student’s transcript as a Pass/Fail PE course.

non-school sports & Activities
This attachment is for students involved in a non-school sponsored sport or activity for which we don’t have a sponsor or coach on campus. Counselors will send these to Andrew Martin for approval. Students should request the waiver packet prior to beginning the activity and submit the completed forms to their counselor signed by the coach/instructor upon completion of the documented hours.
